How to Claim
NASCAR Powerball - The Chase Florida Lottery Semi-Finalist
To claim the official spot as the Florida Lottery Semi-Finalist, the selected player must submit to the Florida Lottery the following documentation:
- Completed and signed Winner Claim Form. English Spanish
- Completed DOL 474 NASCAR Winner Release, Acknowledgment, and Authorization form.

Copy of acceptable identification that is current or was issued within the past five years and bears a serial number
or other identifying number.
Acceptable forms of identification include:
- An identification card or driver's license issued by a public agency authorized to issue driver's licenses in Florida, a state other than Florida, a territory of the U.S., Canada, or Mexico.
- A passport issued by the U.S. Department of State.
- A passport issued by a foreign government.
- An identification card issued by any branch of the U.S. Armed Forces.
- An identification card issued by the United States Bureau of Citizenship and Immigration Services.
- Veteran health identification card issued by the United States Department of Veterans Affairs.
The required documentation may either be delivered to one of the Lottery Offices or mailed to:
Should a player use a mailing service to submit documents, the risk of mailing remains with the player. The Lottery is not responsible for delays in mailing or lost/stolen mail.
The required documentation must be received by the Florida Lottery no later than ten calendar days after being notified (by phone or email) by the Florida Lottery that you are the Florida Lottery Semi-Finalist.
NASCAR Championship Weekend VIP Trip to Miami, Florida
Should the Florida Lottery Semi-Finalist be one of the four (4) players, drawn on October 7, 2026, chosen to receive a Trip Prize, the Florida Lottery's Claims Processing Office will notify that player by telephone or email as soon as practical after MUSL has notified the Florida Lottery. If the Florida Lottery is unable to have contact with the player by October 22, 2026, the player will forfeit his/her right to claim the Trip Prize, and the Trip Prize will be forfeited. The player will remain eligible to be entered into the final MUSL drawing held on November 8, 2026.
To claim the VIP Trip Prize, the selected player must submit to the Florida Lottery the following documentation on or before October 22, 2026:
- Completed DOL 474 NASCAR Guest Release, Acknowledgment, and Authorization form.
